2.11 Local Consumables

In this section, please record the total number of local consumables used per participant, per visit.

This list is strictly for general local supplies. Do not include consumables already accounted for in specific procedure sets or lab kits.

You are able to enter data for the following items:

• Disinfectant wipe
• Emergency contact card
• Empty binder
• Face mask
• Gloves, sterile pair
• Needle, green, stainless steel
• Paper consent form
• Patient brochure
• Patient feedback survey
• Patient folder
• Patient “thank you” card
• Syringe, 20 ml

To calculate the total number of units you need to multiply the units used per visit by the total number of participants and the total number of visits.

If an item is used only once (e.g., at screening), adjust the units to reflect that it is not used at every visit.
